Creating & Managing Care Plans

Overview:
Develop comprehensive care plans for patients, including goals, interventions, and discipline assignments.

Steps

1. Access Care Plans
   In a patient’s Detail view, click the Care Plans tab.
   
   

2. Create a new care plan
   
   

   • Click + Add Care Plan.
     
     

   • Enter Title, Start Date, and Assigned Disciplines (e.g., RN, PT).
     
     

   • Define Goals (e.g., “Improve mobility”) and Interventions (e.g., “Daily PT sessions”).
     
     

   • Click Save.
     
     

3. Edit or update
   
   

   • Click the care plan from the list.
     
     

   • Click Edit, modify goals or add new interventions.
     
     

   • Click Save.
     
     

4. Close or archive
   
   

   • When a care plan is complete, change its status to Completed or Archived.
     
     

Tips

• Link care-plan tasks to visits and auto-visits for automatic scheduling.
  
  

• Use Notes to document progress and updates between disciplines.
  
  

Troubleshooting

• Cannot add goals: Ensure at least one discipline is assigned.
  
  

Plan not visible: Check the patient filter and active status.
